ELIGIBILITY

• MassHealth eligible or with Commonwealth Care Alliance, Fallon, Senior Whole Health, or Tufts Health Insurance
• 16 years of age or older
• Unable to live alone due to a medical, physical, cognitive, or mental condition
• Must have a qualified caregiver living in the home
