Eligibility criteria include being covered by MassHealth, Commonwealth Care Alliance (CCA), TUFTS, United Health, and requiring assistance with a minimum of two of the following Activities of Daily Living (ADLs): walking, dressing, toileting, eating, bathing, and transferring.
