Adult Foster Care (AFC) is a program funded by MassHealth, designed to provide in-home support to individuals who qualify. To be eligible for AFC, individuals must be 16 years of age or older and require assistance with activities of daily living, which is provided by a live-in caregiver. AFC members may have the option to stay alone for up to three hours a day, subject to approval by the AFC Team.
Eligibility for the program necessitates that the member has a medical or cognitive condition that requires assistance. Support services may include:
- Active Supervision and Assistance: Ensuring the member’s health and well-being through active supervision and physical assistance with tasks such as bathing, dressing, toileting, mobility, and eating.
- Medication Management: Reminders, scheduling, and assistance with medication, as well as transportation provision for appointments.
The AFC program has been a part of MassHealth for over 50 years, emphasizing program options that prioritize the Community First principle, allowing individuals to remain in their community whenever possible. We align with MassHealth’s commitment to the importance of family living, recognizing its pivotal role in enabling individuals to lead fulfilling lives.
